HOME VISITS:
Home visits are primarily reserved for patients who are terminally ill or housebound, ensuring that we can provide the highest quality care to all our patients. If you request a home visit and do not meet these criteria, your request will be carefully reviewed by a clinician, who will decide the most appropriate team member to visit based on your medical needs.
Requests for home visits at Wembley Park Medical Centre are received via telephone and online consultations.
All housebound patients have a named GP for routine reviews. We work with the community rapid response team and district nurses for urgent home visits or blood tests. For palliative patients we aim to ensure the practice GP team visit regularly and coordinate with the community teams as well for joint visits where helpful.

We will ask the members of this group some questions from time to time, such as what you think about our opening times or the quality of the care or service you have received.
